Our Service Delivery Plan
Communicare’s service approach is focussed on finding you suitable pathways to successful
employment or training. With the appropriate support we will discuss with you your interests,
strengths and needs and quickly find you your personalised pathway to success.
We provide easy access to our Transition to Work (TtW) services through two full-time sites in
Cannington and Armadale serving you from 9am to 5pm. We are located in modern and safe facilities
close to public transport, with easy access to parking and are accessible to people with disability.
What Support Can You Expect?
All of our staff have experience and are passionate about working with young people to support
them on their journey to employment or education. Our TtW office spaces are specifically designed
to provide our participants with a welcoming, safe and youth-friendly environment. The following
phases will ensure that you are job ready and assist you to secure employment.
1. Engagement and Referral
When you are referred to TtW you will receive a call from one of our friendly staff within 48 hours
to book your initial appointment where you will meet your Youth Mentor and receive a TtW
welcome pack. Your Youth Mentor will support you throughout your TtW journey, start to finish.
Their role is to help motivate and inspire you to work towards identifying and reaching your
employment or educational goals. Youth Mentors can also help you access additional support
services in areas including mental health, addiction and substance misuse, domestic and family
violence, housing and legal matters.
2. Assessment and Commencement
Your Youth Mentor will assess your strengths, transferable skills and challenges to help identify
suitable jobs or education pathways. Together you will create your own Job Plan to help you work
towards your goals. The Job Plan will also include our commitment to you to support you on your
journey.
3. Activities and Engagement
When you commence in to TtW you will have access to a range of practical and learning activities
that will help you build your skills and work readiness. These include;
• intensive and individualised support to address needs/barriers (from both your Mentor and
Employment Coach)
• personal development and team building activities to develop work social skills
• workplace orientation training and work experience
• literacy and numeracy skills, and
• computer skills training
• vocational advice and planning
• acquiring labour market knowledge; e.g. labour hire demands and skill shortages
• structured work experience and community work
Throughout your personalised program we will provide you with appointment reminders through
your preferred method of communication.

4. Job and Education Placement
Once your Job Plan has been complete you will be introduced to your Employment Coach who
will also support you throughout your time in TtW. Your Employment Coach will help you increase
your work readiness skills through a number of activities including;
• learning about job search techniques, career options and employment programmes
• job preparation training (e.g. resume writing, interview and presentation skills)
• entering resumes on job finding websites
• explaining what to do when matched with a vacancy
• provide access to phones and computers for job search
• job interview training techniques
• support to explore educational pathways if further training is needed.
5. Support in the Workplace and Education or Training
Once you have found employment or started in an education course your Youth Mentor and
Employment Coach will continue to support you. This includes regular phone calls, meeting or
workplace visits to help you get the most out of your new job or study. Other strategies to offer
support in the work place will include:
• Contact with your new Employer to confirm negotiated supports, inductions and training
• Provide your new Employer with a TtW information pack
• Assist with any communication, expectations and feedback
• Provide advice on government subsidies to assist with training and development
• Contact your Education Provider to offer support and advocacy throughout your study
• Provide additional support to help you complete your education course e.g. ensuring you
have the right tools to complete your study
• Help you manage and maintain your work, study and personal life effectively.
Our Networks
Communicare has over 30 years’ experience placing people in employment and our staff are highly
skilled in connecting the right person to the right job. Communicare has an extensive network of
employers to broaden your employment opportunities and support you to secure work. Some
examples of employers include:

• CCI VTEC
• Working Careers
• National Workforce Network
Our Employment Coaches are highly skilled in approaching employers and advocating for employment
opportunities for young people. We also promote our professional post-placement support and
Youth Wage Subsidies.
Our subcontracting partner, the Wirrpanda Foundation will also provide specialist support to
Indigenous participants which will assist in finding suitable employment, training or other activities.

Communicare has over 30 years’ experience placing people in employment and our staff are highly
skilled in connecting the right person to the right job. Our staff are passionate about creating
sustainable employment outcomes through ensuring our job seekers and well equipped with
everything they need to know before entering the workforce.
There are many benefits to hiring young people participating in the TtW;
• All of our young people we place in employment will be work ready and aware of what is
expected of them by employers
• Youth Wage Subsidies are available for eligible candidates
• Intensive post-placement support will be offered by our experienced Employment Coach
• Job Seekers who are enthusiastic about work with an eagerness to learn
