If I want to have the option to retest, I must turn in this contract with the attached Study Plan within two (2) school days of getting my test score back. (Or one weekend if given back on a Friday) 3. I need to prepare carefully for this reassessment opportunity. The maximum grade I may earn is the cut off score of 75%. 4. I am given the opportunity to retest an assessment only once. 5. I, along with my parents, am responsible for arranging transportation to any and all tutoring sessions I am scheduled to attend. 6. If I earned a failing grade due to cheating on a test, I am ineligible to participate in the reteach, retest process and the infraction will be handled in accordance with the school districts policy. 7. Retests will not be offered during the last week of school, during the last week of a marking period and for certain assessments due to the nature of the assessment. In order to take a retest, I understand that I will have to complete the following: 1. I will, on my own time, complete the required self‐reflection requirement and bring it to the retesting session. (Each answer should be on a separate sheet of paper, no less than 5 sentences and stapled to this form.) a. Write a paragraph explaining your commitment to the following areas: Attendance, homework, class participation, and daily studying/reviewing. b. Write a paragraph explaining what you can change to make a difference in your preparation for this retest opportunity and future test. c. Write a paragraph explaining how your teachers/parent/guardian can help you. 2. I must report to at least 2 tutoring sessions. a. My teacher will announce/post dates and times when these sessions will be held. b. Attendance at each session must be documented on the daily study log. c. Reporting on the specified day to take the reassessment (Retest) does not count as a tutoring session. d. Failure to report to both of these tutoring sessions will cause me to lose the opportunity to retest.
