Comprehensive Vascular Disease
Prevention and Management Initiative
A local integration project with provincial application.
The leading cause of death among Ontarians is vascular
disease. The burden of this disease and its related
conditions within our aging population has catastrophic
implications for the ability of the Provincial health system
to meet the current and emerging health care needs
of Ontarians.
As the vascular system is a common element to a wide
variety of clinical conditions involving targeted organs;
it is essential that a strategy be developed to aid in the
prevention, detection and management of a disease
process that affects this system.
GOAL
To develop and successfully implement a vascular
prevention, early detection and management strategy
in the Central East LHIN that improves the health of
the population, patient outcomes and experience,
reduces downstream costs and is transferable to
other communities across Ontario.
Purpose
• A comprehensive regional vascular prevention
and management strategy will be implemented to
co-ordinate the identification and treatment of
modifiable risk factors for vascular disease including,
hypertension, dyslipidemia and chronic kidney disease
for attached and unattached patients across the
healthcare continuum
• The vision is a patient-centred approach that will
integrate primary care, community based specialty
care and in-hospital care and will coordinate screening,
prevention, diagnosis, treatment, medical management,
follow up and patient self management
• This program will effectively leverage existing resources
and infrastructure within the Central East LHIN to
implement clinical practice guidelines-based processes
and protocols to improve the access to and provision
of timely and responsive, patient-centred care
• The strategy will achieve improved patient outcomes
and experience and will demonstrate return on investment
with the reduction of downstream costs in line with
provincial priorities
