Comprehensive Vascular Disease Prevention and Management Initiative A local integration project with provincial application. The leading cause of death among Ontarians is vascular disease. The burden of this disease and its related conditions within our aging population has catastrophic implications for the ability of the Provincial health system to meet the current and emerging health care needs of Ontarians. As the vascular system is a common element to a wide variety of clinical conditions involving targeted organs; it is essential that a strategy be developed to aid in the prevention, detection and management of a disease process that affects this system. GOAL To develop and successfully implement a vascular prevention, early detection and management strategy in the Central East LHIN that improves the health of the population, patient outcomes and experience, reduces downstream costs and is transferable to other communities across Ontario.
