101 Recognizing and Reacting to Abnormal Operating Conditions 101 Recognizing & Reacting to AOC’s Performance Checklist [Revision – 0] [Revision Date 05/28/01] Operator: Operator Address: Candidate Name: SS# / Employee Number: Verifier’s Name: Directions: This Performance Checklist details the knowledge, skills and abilities to perform the job located in the header of this document. Prior to unsupervised performance of any task associated with or operating equipment detailed on this document, the operator must be signed off by a subject matter expert (SME)/Verifier. For a candidate to be qualified for any task, an individual must demonstrate a score of 80% of the total knowledge requirements of this checklist. Meeting the knowledge requirement, the individual may then proceed to the Performance section of the checklist. A candidate is required to complete all performance items with 100% accuracy on the Performance section of this checklist and complete all AOC items with 100% accuracy on the Abnormal Operating Conditions section of this checklist for a verified status. Recommended Training materials and references: 1. eWebOQ Module 101 Recognizing and Reacting to Abnormal Operating Conditions 2. DOT 49 CFR 192 Subpart N and 195 Subpart G Regulations 3. DOT 49 CFR 191 Regulations 3. Company Operating Procedures Step 101 K10 Knowledge Verifier’s Signature KNOWLEDGE SECTION OF THE PERFORMANCE CHECKLIST (The candidate must respond to the following knowledge statements to the satisfaction of the verifier.) Define “Abnormal Operating Conditions (AOCs)” as identified by the DOT’s Operator Qualification Rule in Subpart N in 49 CFR Part 192 and Subpart G in 49 CFR Part 195. • “A condition identified by the operator that may indicate a malfunction of a component or deviation from normal operations that may indicate a condition exceeding design limits or result in a hazard(s) to persons, property, or the environment.” Remarks: Date: Define “Qualified” as outlined in the DOT Operator Qualification Regulations. 101 K20 Remarks: Date: Define “Covered Tasks” as outlined in the DOT Operator Qualification Regulations. 101 K30 Remarks: Date: Copyright © 2001. eWEBOQ. Page 1 of 13 101 Recognizing and Reacting to Abnormal Operating Conditions 101 Recognizing & Reacting to AOC’s Performance Checklist [Revision – 0] [Revision Date 05/28/01] Step 101 K40 Knowledge Describe, “react” as it pertains to dealing with AOCs. Verifier’s Signature Remarks: Date: State the difference between an “AOC” and an “Abnormal Operation”. 101 K50 101 K60 101 K70 Remarks: Date: List and discuss the appropriate actions to abnormal operations a transmission operator should consider: • The type of operations. • The location where the condition exists (i.e., proximity to public). • The nature of the condition (i.e., the extent to which it could lead to an emergency situation if not immediately corrected). • Its effect on the operation of the operator's system. Remarks: Date: Describe the DOT (49 CFR 192. 605 and 195.402) Regulations for Abnormal Operations: • 192.605[c] applies to transmission pipelines; it does not apply to distribution systems. • 195.402[d] applies to hazardous liquids. • 192.605[c] and 195.402 [d] require procedures to provide safety, when operating design limits have been exceeded - responding to and investigating and correcting the causes of: • Unintended closure of valves or shutdowns; • Increase or decrease in pressure or flow rate outside normal operating limits; • Loss of communications; • Operation of any safety device; and, • Any other foreseeable malfunction of a component, deviation from normal operation, or personnel error, which may result in a hazard to persons or property. Remarks: Date: Describe the extent of follow-up monitoring after the end of an abnormal operation. 101 K80 Remarks: Date: Copyright © 2001. eWEBOQ. All Rights Reserved. Page 2 of 13 101 Recognizing and Reacting to Abnormal Operating Conditions 101 Recognizing & Reacting to AOC’s Performance Checklist [Revision – 0] [Revision Date 05/28/01] Step 101 K90 Knowledge Identify the following anomalies, which should be recognized by personnel who perform operation and maintenance activities as potential safety-related conditions that may be subject to the reporting requirements of DOT (49 CFR 191.23). • General corrosion that has reduced the pipe wall thickness to less than that required for the maximum allowable operating pressure. • Localized corrosion pitting which has progressed to a degree where leakage might result. • Unintended movement or abnormal loading by environmental causes, such as an earthquake, landslide, subsidence or flood that impairs the serviceability of a pipeline segment. • Material defects, such as those caused in the manufacturing process, or physical damage that impair the serviceability of a pipeline segment. Sound engineering criteria should be used to determine if an observed condition involving a material defect or physical damage impairs serviceability. • Malfunctions or operating errors that cause the pressure of a pipeline to rise above its maximum allowable operating pressure plus the build-up allowed for the operation of pressure limiting or control devices. • Leaks in a pipeline constitute the need for immediate corrective action to protect the public or property. Including: Leaks occurring in residential or commercial areas in conjunction with a natural disaster, leaks where a flammable vapor is detected inside a building, leaks that involve response by police or fire departments. • Other known anomalies or events that could lead to an imminent hazard (and cause, either directly or indirectly, by remedial action of the operator) for purposes other than abandonment, a 20 percent or more reduction in operating pressures or shutdown of operation of the affected pipeline segment. Remarks: Verifier’s Signature Date: 101 K100 Discuss why special consideration should be given to the information obtained through the use of an instrumented (smart) pig. Remarks: Date: 101 K110 State the date to be used to determine the filing deadline for reporting an anomaly as per DOT (49 CFR 191.25). Remarks: Date: 101 K120 Describe the abnormal operating condition rating system for documentation and, ultimately, for repair: • Type A – Imminent Hazard • Type B – Potential Hazard • Type C – Non-Hazardous Remarks: Date: Operator Specific Knowledge: 101 K130 101 Remarks: Date: Operator Specific Knowledge: Copyright © 2001. eWEBOQ.
