We are pleased to present the 18th annual University of South Florida Geriatric Health Care Conference, sponsored by
the USF Health Byrd Institute. This continuing education conference is designed to provide practitioners, administrators and
service providers current information on Alzheimer’s disease and other critical issues in aging.

From I-275 traveling North (from Tampa International Airport) or traveling South, take exit 51 for Fowler
Ave. Travel East 2 miles towards University of South Florida. The Embassy Suites Tampa Busch Gardens
hotel is on the left after the intersection of Fowler and 30th street.
From I-75: From I-75 traveling North (from Sarasota/Ft. Myers/Naples) or traveling South (from Gainesville/Ocala),
take exit 265 for Fowler Ave. Travel West 4 miles towards Temple Terrace/University of South Florida. The Embassy Suites Tampa USF Busch Gardens hotel is on the right after the main entrance to USF.
From I-4: From Orlando take I-4 West to I-75 North and take exit 265 for Fowler Avenue and follow
directions above.
CONTINUING EDUCATION
THE CONFERENCE IS APPROVED FOR 18 HOURS OF CEU CREDIT; 6 HOURS PER DAY.
 The Byrd Institute is approved by the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health
Counseling as a provider of continuing education, and this program is approved for 6 hours of continuing education for each
day.
 The Byrd Institute was approved by the Florida Board of Nursing Home Administrators to offer this program for 6 hours of
credit per day for nursing home administrators.
 The Byrd Institute will provide certificates for assisted living facility administrators.
 The Byrd Institute was approved by the Florida Board of Nursing to offer this program for 6 hours of credit per day for nurses.
 The Byrd Institute was approved by the Florida Board of Occupational Therapists to offer this program for 6 hours of credit per
day for occupational therapists.
 This program was submitted to the Florida Statewide Public Guardianship Office for 6 hours of continuing education units
per day and a maximum of 18 hours of continuing education units for the entire program.
 This program was approved by the Florida Council of Dietetics and Nutrition for 6 hours of continuing education units per
day.
CERTIFICATES AND CE BROKER
 Continuing education credit is approved on a per day basis. Participants must attend the entire day to receive credits. No
partial credit will be given.
 All participants will receive a certificate of attendance for the day(s) they attend. Participants will also receive a certificate
specific to their discipline documenting the CEU credit they received.
 Participants must sign in each morning and sign out in the afternoon on the sign out sheet for their discipline.
 Please be sure to bring your license number with you to write on the sign out sheet.
 Participant’s Continuing Education Units will be entered on the CE Broker website within two weeks following the conference.

CANCELLATION POLICY
All cancellations must be submitted in writing to the USF Health Byrd Institute at 4001 E. Fletcher Ave, Tampa, FL
33613. Cancellations received by April 30, 2016 will be granted a 100% refund. Cancellations received by May 31, 2016
will be granted a 50% refund. NO REFUNDS OR CREDITS will be issued after May 31.
Registration is from 8:00 – 8:30 each morning; coffee and refreshments will be served.
The conference will be held 8:30am - 4:30pm daily.
