Description of the Purpose and Course Content: To provide supervised clinical experiences* in varied clinical sites.* Clinical preceptors are community based and not course faculty. Student Learning Outcomes: Upon completion of the course the student shall demonstrate an ability to perform the following behaviors in the clinical setting: 1 Apply theory scientific principles and current research findings underlying obstetric and gynecologic diagnostic and therapeutic with procedures in the management of health care of women throughout the lifespan 2. Elicit and document an appropriate health history including an obstetric and gynecologic history, using effective interviewing techniques, with emphasis on genderbased differences. 3. Perform an appropriate episodic and / or complete physical examination on women. 4. Assess for disease risk factors specific to women. 5. Order, safely perform procedures, and accurately interpret select diagnostic and screening tests with women 6. Demonstrate critical thinking in formulation of differential diagnoses and management based on history, physical examination and clinical and diagnostic test findings. 7. Diagnose and manage prenatal, newborn*,intrapartal*, well woman, postpartum and common primary care problems with emphasis on gender specificity using evidencebased guidelines and current research findings. 9. Accurately and legibly furnish pharmacologic and non-pharmacologic agents with the preceptor with special consideration to pregnancy, lactation, socio-cultural background, and financial resources. 10. Incorporate current health promotion, health protection, and disease prevention recommendations into the clinical practice. 11. Provide appropriate client education, counseling, and anticipatory guidance in the implementation of the advanced practice role. 12. Identify appropriate community resources / agencies for client referral. 13. Apply research that is women-centered and contributes to positive change in the health of women or the health care delivered to women. 14. Midterm and final week counseling for submission of portfolio. 3 Scored activities and weighting by percentage of total score: Documentation (charting of SOAP Notes) 25% Faculty & Preceptor Evaluations 75% Final 07/27/14 LPH Final course grades are based on faculty and preceptor evaluations of clinical performance and documentation (charting) submitted during the semester. EVALUATION METHODS: Clinical Evaluations: Faculty has the primary responsibility for student grading. Written evaluations based on course outcomes will be sought from clinical preceptors. Students will be evaluated by their clinical preceptors and by course faculty. 1. Preceptor Evaluation: A student is evaluated by each preceptor in written formatt a minimum of once each clinical shift. Students are responsible for requesting from clinical preceptors and submitting completed evaluations to course faculty on the announced due date. Evaluations should reflect the variety of clinical experience at each specific clinical site. Make sure each evaluation designates the type of patient seen and preceptor name!! A summative end of semester evaluation is also submitted by each clinical preceptor. 2. Faculty Evaluation: A student will be visited by a faculty member in each clinical setting during the semester. Faculty will schedule clinical visits with the student and will observe as possible, the student examining patients, presenting patients to the preceptor, and devising a treatment plan. Faculty will schedule conferences with the student to discuss his / her clinical performance during the semester. 3. Satisfactory evaluations (complete number, variety and passing scores) at the end of the semester are required for successful course completion. Clinical Assignments 1. Students will complete and submit the following documentation by email weekly every Sunday evening by 1000 pm: a) TYPED Practicum log – record of hours and activities b) TYPED Clinical (STAT Sheet) c) Patient Encounter Form. Make a list of all patients seen each day with age and diagnosis such as: 26 yo G2P1 at 36 weeks/r/o PIH Or: 42 WW Exam, Depo and Pap d) Charting (Documentation from the Medical Record): Students will select and submit a copy of one charting without patient identification (SOAP NOTE) every two weeks for 8 4 Final 07/27/14 LPH total. Charting selected for submission throughout the semester should be representative of all clinical settings/experiences. At midterm and final counseling each student must submit a hard copy of all clinical evaluation and assignments for review that will be kept by the course instructor. Grading Scale: To successfully complete the course you must receive an average of C or greater. Students who have been admitted with special academic requirements for successful course completion should notify course faculty at the beginning of the semester. (This must be done as soon as the student knows she will not be attending clinical (even if it is 5 am in the morning= text professor, followed by confirmation email!!!) Clinical attendance is expected. If you are unable to attend clinical you must notify faculty (Dr. Hunter: lhunter@mail.sdsu.edu) prior to clinical and also your preceptor. You will be required to complete a make-up assignment. You may NOT LEAVE a clinical once present until notifying Dr. Hunter. If you are ill, you may be required to submit medical documentation. Work schedules are not valid reasons for missing clinical. You should not plan vacations, CE conferences, elective surgical procedures etc. that conflict with clinical time. Clinical absence, leaving clinical or tardiness WILL result in a lowering of the course grade or in course failure. You will be expected to be on time for clinical. Late arrivals are disruptive and discourteous to your preceptor and others. Cell phones should be silenced and text messaging is not permitted during clinical time. Educationally prepared with a Master’s degree in Nursing (preferred), or a highly experienced nurse practitioner with a baccalaureate degree in nursing who is recognized as a clinical expert and is supervised by a Master’s prepared practitioner. Willing to meet with faculty and participate in an orientation to the SDSU School of Nursing 9 Final 07/27/14 LPH Advanced Practice Program. Able to provide a clinical practice setting that serves well-women and gynecological clients and facilitates student learning and achievement of course objectives. Able and willing to precept the student in the clinical setting. Willing to critically evaluate the student’s clinical performance in writing during and at the end of the semester. Willing to be evaluated by the student. Willing to meet with SDSU faculty during periodic site visits to discuss the student’s progress. Preceptor Responsibilities The preceptor agrees to: Orient the student to the clinical setting. Provide sufficient space and time to allow the student to see patients. Provide the student with access to the kind of patient encounters that are needed to achieve the student’s learning objectives. Provide supervision and validation of the accuracy of the student’s history, chart review, and physical examination findings and her / his assessment of the patient’s presenting condition. Review the student’s tentative plan for management of the patient’s health promotion needs and / or presenting conditions and provide necessary guidance to ensure appropriateness and comprehensiveness of the care plan. Supervise the student when she / he initiates patient management regimens and referral procedures. Meet with program faculty who make periodic site visits to review student progress and share ideas concerning ways to facilitate student learning. Maintain communication with the program faculty as necessary. Complete one standardized evaluation form for each 6 hours of clinical practice. Based on your experience as a preceptor, share ideas for improving the clinical component of the program with program faculty. Student Responsibilities: The student shall: Identify his / her learning needs and communicate them to the preceptor. Adhere to assigned days and hours. If unavailable to assigned time, inform the preceptor and faculty before the clinical experience. 10 Final 07/27/14 LPH Seek new clinical experiences that will develop his / her assessment, diagnostic, and treatment skills. Work with other team members within the organization. Demonstrate an increasing degree of independence. Provide ongoing feedback to the preceptor regarding the clinical experience. Seek preceptor evaluations at designated times during the semester. Adhere to all agency / office regulations. Adhere to all School of Nursing policies regarding immunization, CPR and professional liability insurance. Faculty Responsibilities The faculty will: Identify and evaluate clinical sites to facilitate student learning. Identify, interview, and evaluate clinical preceptors. Assign students to clinical practicum sites to meet course objectives and individual learning needs. Periodically visit the clinical site to engage in clinical teaching, to obtain feedback from the student and preceptor, and to evaluate the student’s clinical performance. Review preceptor evaluations to assess student learning and progressive development in the role. Assign final course grades.
