INFORMATION
1. You have been asked to participate in an evaluation study of student experiences in CommUniverCity
San José.
2. You will be asked to complete a short survey evaluating your experiences participating in a
CommUniverCity San José project this semester.
3. The survey should take no more than ten minutes to complete.
4. This survey is confidential, but not anonymous. You received a unique link to this online survey that is
linked to your email address, and your IP address is also recorded when you complete the survey in
order to track your participation. The survey does not ask for any other identifying information,
however, and email addresses and IP addresses will be removed from the final dataset before any
analysis takes place. No identifying information will be used in any part of the analysis or reporting of
results from this survey.
RISKS
There is a risk that some of the questions may be on topics you do not feel comfortable answering
questions about. You may skip any question that makes you uncomfortable. There are no penalties for
skipping questions.
BENEFITS
There are no anticipated benefits to participating in this study. However, your participation in this
research will help in our ongoing evaluation and improvement of CommUniverCity San José programs.
CONFIDENTIALITY
The information in the study records will be kept strictly confidential. Data will be stored securely and
will be made available only to persons conducting the study unless you specifically give permission in
writing to do otherwise. No reference will be made to your identity in oral or written reports which
could link you to the study.
COMPENSATION
There is no compensation for participating in this study.

PARTICIPATION
Your participation in this study is voluntary. You may refuse to participate in the entire study or in any
part of the study. You do not have to answer any questions that you do not want to answer. If you
decide to participate in the study, you are free to withdraw at any time without any negative effect on
your relations with San José State University or with any other participating institutions or agencies.
