Please complete the form, print it, scan and e-mail it to isss@mnsu.edu. The form cannot be saved after you complete it, it
MUST be printed, scanned, and e-mailed. Your I-20 will be sent within 10 business days after the completed form is received
by the International Center. To ensued the accuracy of this document, please include a scanned copy of your passport
identification page with the I-20 Request Form in email.

If your I-20 needs to be sent outside of the United States, we STRONGLY URGE you to choose to use eShip, which will minimize
the risk of loss and speed delivery of your documents. To use eShip, sign up at https://study.eshipglobal.com/ and follow the
directions. If you do not sign up for eShip, your documents will be sent by postal mail and may take 3-5 weeks to reach you. If the I20 is to be shipped out of the U.S. but you complete this form less than 90 days before Orientation start, ESHIP is REQUIRED.
