Academic Data Release Agreement
By completing this agreement, I authorize the Assistant Director of Student Activities for Greek Life to view my
academic data and verify my eligibility for membership in a fraternity/sorority at Minnesota State University,
Mankato. I understand that my enrollment status, class standing, and grade point average will be provided to
the Interfraternity Council or Panhellenic Council chapter(s) requesting such information.
I understand that, unless otherwise specified, this agreement shall remain in effect throughout the term of my
enrollment at Minnesota State University, Mankato. I also understand that I may terminate this agreement, at
any time, by notifying the Assistant Director of Student Activities for Greek Life of this desire in writing.
