Statement of Continued Education For Adoption Subsidy Recipients

His or her anticipated completion date (Month/Year) is __________________.
I understand that the continuation of Adoption Subsidy is conditioned on the Division’s
determination that the home schooling program in which my child is participating is a secondary
school equivalent as required by N.J.A.C. 10:121-1.4. I have attached to this form evidence of
such equivalency, which may include:





A written description of the program
A description of its curriculum
Reading lists
Objectives
Any other documentation that may assist the Division in making this determination.
I understand that the continuation of Adoption Subsidy beyond this child’s 18th birthday is
conditioned on the continual enrollment and participation in a secondary education program or
its equivalent. I agree to promptly notify the Division of Child Protection and Permanency,
Office of Adoption Operations of any change to the information provided here.
I attest that all information provided here and on any attachments is correct to the best of my
knowledge, and understand that willful misrepresentation of facts may jeopardize receipt of
Adoption Subsidy.
