Dear [Enter name of adoptive parent's attorney]:
We have completed the investigation in the above matter for purposes of the Preliminary Hearing
and attach herewith a copy of our report which was mailed to [Enter name of Surrogate].
We have received an Order from the court appointing us to supervise the adoption. We are
attaching a Statement of Costs for services in regard to supervision and the Final Report to the
Court. The check should be made payable to the “State of New Jersey” and sent to this office at
least two weeks prior to the Final Hearing.
We have completed our supervision in the above matter and are sending our report and Consent
to the adoption to [Enter name of Surrogate].
