Proficio Course Application Form

Your application will be sent to the Director of Research Training for consideration. If the Director of
Research Training supports the application, he will liaise with the appropriate Head of Department (or
their deputy) prior to final approval. You will be contacted with the outcome of your proposal.
Please note that information supplied on this application will be used in the course approval process
and also in publicising the course on the Proficio website.
