4-H Needs Your Thoughts!
We need your help and feedback! As a young person who has been involved in 4-H in the past, you have
a unique perspective to help us. We value your opinions. We are asking you to share your thoughts with us
through this short survey.
This survey is for 4-H members who have graduated from 4-H AND 4-H members who have chosen not to
re-enroll in 4-H. Your feedback will help us address reasons which may have influenced your decision to
not re-enroll. Youth are encouraged to complete this survey with parents help if needed.
The feedback will be shared with 4-H staff in the state. Your participation in this evaluation is voluntary and
confidential to the extent allowed by law. Your responses will be combined with the responses of all other
participants and you will not be individually identified on any report prepared. If you have questions, please
