Dear Parents of Rising SFMS Sixth Graders:
We are very excited that your child will be a sixth
grader at South Forsyth Middle School next year!
Each of our incoming sixth graders will receive an
SFMS T-shirt when they tour our school this spring.
Our hope is that by having a common T-shirt all of our rising sixth grade students will
feel like they are a part of one big SFMS family. We are excited to be able to provide the
T-shirt to your student as a gift from SFMS.
Please complete the bottom portion of this form, indicating your child’s size. Please keep
in mind that this is a T-shirt that your student will be wearing all of his/her middle school
years. You might want to order a bit larger than what your student currently wears.
