Continuous Improvement Goal:
South Forsyth Middle School will improve student achievement and student success through the
implementation of the Data Team Process, Quadrant D Learning, and the 7 Mindsets character education
program.
SMART Goal:
SMART Goal: The percent of students with typical or high growth will increase 4 percentage points from the 2013-2014 to the
2015-2016 school year as measured by the CCRPI Progress Score.
Language Arts: 66.84 % to 70.84 %
Mathematics: 74.94 % to 81.94 %
Science: 68.71 % to 72.71 %
Social Studies 76.25 % to 80. 35 %

The teachers will work collaboratively with
their grade and content alike peers to
develop common assessments, analyze
data and implement instructional
strategies for various groups of students
to maximize learning. Teachers will
continue to attend professional learning
opportunities to grow in the effective use

activities and projects with a focus on
providing high rigor and relevant
(Quadrant D) learning experiences for
their students. Teachers will participate in
ongoing professional learning based on
the work of Bill Daggett and the
International Center for Leadership in
Education.
Teachers will work collaboratively with
their grade level to create an “Expo Day”
three times per year which requires crosscurricular planning for the day’s activities.

7 Mindsets
Teachers will provide year-long lessons on
the 7 Mindsets character education
program. Students will interact with their
teacher and peers to discuss the weekly
topics and journal their reflections.
The themes of the 7 Mindsets program
will be interwoven throughout the daily
business of SFMS so it becomes a part of
the culture of the school.
Students will internalize the values of the
program as they learn to be 100%
accountable, connect to others, serve
others and appreciate what they have
been given.
If students participate in the 7
Mindsets program, then they will
learn mindsets to help them be
strong individuals willing to be
involved in making a positive impact
in the world.

Describe how your SMART Goal will be monitored throughout the year:
Teachers will analyze their Data Team assessment data four times throughout the year to monitor student progress, identify
students’ strengths and weaknesses, and design learning opportunities for different groups of students based on the assessment
results. The administrative team will attend Data Team meetings to review the results and participate in the discussions of
instructional strategies to maximize learning. Professional learning opportunities will be designed as needed to support the Data
Team Process and the implementation of instructional strategies. Teachers will submit to the administration Data Team Assessment
Results Reflective Questions. The reflective responds will be discussed at the mid-year TKES conference and the end-of-the-year
TKES Conference.
Quadrant D (high rigor and relevant) learning experiences will be imbedded in the daily lessons. Teachers will be required to share
with the administration lesson plans that exhibit the characteristics of Quadrant D learning. Additionally, teachers will plan a crosscurricular “Expo Day” three times this year which will provide students with interconnected lessons from all content areas.
The 7 Mindsets program will be monitored through analysis of pre-implementation and post implementation student surveys.
