AUTHORIZATION AND RELEASE
By my signature below, I grant authorization and permission for my son/daughter (Student) to attend the Ski and Board Club trips to
Seven Springs Mountain Resort in Champion, PA. The Undersigned parents or guardians and student are aware that any sport or
activity involves some risks, and understand that participation in the Ski and Board Club, or travel on Ski and Board Club trips to
mountain resorts, is not required, but rather is voluntary. The Undersigned assume all risks of harm, including any risks of property
damage and/or personal injury to Student, which may arise out of this activity. In consideration of the agreement of the Ski and
Board Club and the School District to include the Student in Ski and Board Club activities and/or travels relating to the Ski and
Board Club, the Undersigned hereby releases the Ski and Board Club, the faculty sponsors, any other sponsors or chaperones, the
North Allegheny School District, and all current and former directors, officers, employees and agents of the North
Allegheny School District, (collectively "Releasees"), from any and all claims, liabilities, demands, actions, causes of action,
expenses and attorney’s fees, whether for negligence or otherwise, and/or for personal injury, property damage or otherwise, arising
directly or indirectly out of participation in Ski and Board Club activities, including travel (collectively "Claims"). In further
consideration of the agreement of the Ski and Board Club and the School District to include the Student in this activity, the
Undersigned also hereby agrees to indemnify, defend and hold harmless the Releasees from any Claims which any of the
Undersigned cause or are involved in, and to reimburse the Releasees for any losses, damages, costs, or expenses, including
attorney’s fees, which any Releasee may incur by reason of any such Claim.
It is hereby agreed that the student must abide by all applicable North Allegheny School District rules as well as the local laws and
ordinances of Champion, PA, and the Seven Springs Mountain Resort. If my son/daughter fails to abide by any of these rules,
he/she may be removed from the trip without being entitled to a refund. Furthermore, he/she may be subject to North Allegheny
School District disciplinary proceedings and/or local law enforcement proceedings.
The activities of the Ski and Board Club are open to all North Allegheny Intermediate High School students regardless of skiing
knowledge or ability. It is strongly recommended that all beginning skiers and snowboarders sign up for a lesson offered by
the ski resort by qualified instructors.
The Undersigned agree that they are responsible for Student's health/medical insurance coverage, and that neither the School
District nor the North Allegheny Intermediate High School Ski and Board Club provide such coverage for students. The
Undersigned further hereby authorize any necessary medical treatment of Student in the event that he or she sustains or incurs an
injury or illness during in or in connection with this activity, including travel, and guarantee payment for all charges arising from
such treatment.
NOTE TO PARENTS/GUARDIANS AND PARTICIPATING SKIERS/SNOWBOARDERS
1. Consuming or using alcoholic beverages and/or drugs will NOT be permitted on the bus or at Seven Springs. Any student
found possessing, transmitting, or under the influence of drugs or intoxicants will be disciplined under and according to the NASD
Discipline Code. Consequences for violations of the Discipline Code may include suspension and/or expulsion from school. All
students are also subject to all rules and regulations of the Seven Springs Police Force during the activity, and the school or its
representatives do not have any power to interfere with official police action.
2. Smoking or chewing tobacco IS NOT PERMITTED.
3. The parent/guardian will have to pick up and transport their child home from Seven Springs if the chaperon feels that is necessary
because of rules that have not been followed.
