Please complete this application, and return it to the UNC School of Medicine,
Office of Special Programs, via email attachment with attention line “MED TA
Application” to medprogramunc@gmail.com
Completed applications are due on Friday, January 8, 2016 by 4pm.
Applicants will be notified about scheduling interviews in January 2016.
Instructions for scheduling interviews will be included in emails notifying students
about the status of their application.
