Confidentiality: Confidentiality of counselor/student and medical provider/student relationships and records is maintained in a manner consistent with accepted professional standards and with local, state, and federal laws. Counseling and medical visits are documented in a secure electronic medical record system which meets healthcare security standards. The counseling and medical staff work together to provide students with integrated health care. Certain information from your medical record, such as appointment date(s), provider name(s), diagnoses, allergies and medications, will be available to all counseling and medical providers within CHW. Detailed CHW appointment/visit summaries, however, are not shared between CHW medical and counseling staff unless there is a shared treatment plan between medical and counseling providers, to which you have agreed, or one of the exceptions detailed below applies. Information shared in your counseling and/or medical visits may be shared with others within CHW and outside of CHW as appropriate when there is concern about imminent self-harm or harm to others; this may include notifying your family and/or other Suffolk University officials. Such disclosure will be made only: (a) when necessary to protect you or an identifiable third party from imminent physical or psychological danger; (b) in cases of alleged child abuse, elder abuse or abuse of a disabled person; (c) upon your submission of a signed document requesting release of treatment information to a specific party or parties; (d) in order to defend CHW and its employees against a complaint or lawsuit you file against it; or (e) in the rare instance that you may become involved in a court case and the court subpoenas counseling and/or medical records and/or requires testimony which may involve responding to inquiries about your treatment in the Center. Copies of reports and other communication between the Center and outside agencies will be shared with you, upon request.
