User Study Informed Consent Form
You have been selected to participate in a demonstration of the interactive prototype of
Cal Explorer, a Flash-based game. Through this demonstration, we hope to collect feedback
about positive and negative aspects of the Cal Explorer interface.
Should you choose to participate, you will be asked to complete the tasks that we will or
have described to you to the best of your abilities. This will include reading background
information, playing a few trivia games, and adjusting in-game options. You may interact with
the game via the mouse and keyboard.
Your identity will be completely confidential. Only cursory information about your
identity (such as gender, level of education, etc.) will be used. Your name will not be revealed
without your consent.
Should you choose to participate, you may discontinue participation at any time.
