To the Trustees of the California State University, on behalf of the State of California (hereinafter called the
Trustees):
The undersigned proposer hereby offers, in the amount stated below, to furnish all labor, materials, tools,
equipment, apparatus, facilities, transportation, and permits for the design and construction of Project Number
(#), (Project Name), at (Campus), in accordance with all the requirements of the Request for Proposal, and
hereby agrees to enter into contract for Project Number (#) if this offer is accepted by the Trustees.

The above amount is to be stated in figures only, and represents the total amount bid for the entire contract work
including all applicable taxes. Any alteration, erasure, or change must be clearly indicated and initialed by the
bidder. The proposer agrees that if there are any discrepancies or questions in the figures, the Trustees will use
the lower figure despite the proposer’s intent. The proposer agrees that the above prices will be held firm and
fixed for
days in accordance with the proposed calendar contained in the Information for Proposers. The
Trustees reserve the right to reject any and all bids and to waive any irregularities. The construction budget is ${

The bid is subject to the provisions contained in the Design/Build Contract General Conditions (note especially
Article 32.00 et seq. regarding instructions to proposers), and the proposer agrees that failure to comply with the
conditions thereof shall be basis for rejection of this bid.
The Trustees require the successful proposer to achieve three percent (3%) DVBE participation in contracting
construction projects as established in the Request for Proposals. {University shall describe the DVBE incentive
herein.} Proposers shall contact the Trustees' DVBE Coordinator at (telephone no., e-mail address).
The proposal must be submitted on this Cost Proposal Form, completely filled out and in a sealed envelope, and
delivered to (place of bid opening), at (Campus), before (time) on (date), or it will be disregarded. The Trustees
will only accept proposals from prequalified contractors with current California State License Board-issued
(insert classification type) license and current California Department of Industrial Relations Public Works
Registration number. Note: technical proposal documents shall be a separate submittal, in a separate sealed
envelope.
Proposer shall enclose with this Cost Proposal Form bidder's security in the amount equal to at least ten (10)
percent of the total amount proposed, including alternatives if additive, (see Design/Build Contract General
Conditions, Article 32.05-c). If the proposer is awarded the contract and then fails to execute the contract, this
bidder's security shall be forfeited to the State.
