The Student Government Association Executive Council (SGAEC) seeks nominations for the
SGAEC Excellence in Teaching Awards for 2016. Students are encouraged to nominate faculty
members who are most deserving of these awards. The deadline for nominations is February
29, 2016.
Two faculty members from each UT Health Science Center college will receive awards. The
recipients will receive recognition in the following ways: A plaque, a $2,500.00 stipend, and
public recognition at the SGAEC Banquet, College Convocations and Commencement
Ceremonies. Faculty members may not receive an SGAEC Excellence in Teaching Award
two consecutive years. Below is a listing of the 2015 recipients who are ineligible this year.
Please complete the top of this form, and provide comments on page 2 that address the award
criteria. Nominate a faculty member who you feel has:
1) Demonstrated outstanding teaching ability
2) Presented material in a way that increased your interest in the subject and promoted
independent thought
3) Effectively organized and communicated subject matter
4) Responded to students’ needs inside and outside of the formal teaching environment
5) Demonstrated genuine concern about student learning
It is important that your written comments provide a detailed description of how the nominated
faculty member fulfills these criteria. Personal examples to illustrate these points will be
especially helpful to the selection committees.

300, on or before February 29, 2016. Members of a student selection committee in each college
will review the information on the form. All discussions regarding the nominees are kept
confidential. Please write about the following qualities of the individual who has been
nominated by providing comments for each criteria listed. Personal examples will be helpful to
the selection committee. Use one evaluation form per nominee. Please type or print neatly in
ink.
