Parent Signature: PE Teacher: FOLSOM MIDDLE SCHOOL 2015-16 PHYSICAL EDUCATION CLASS PROCEDURES* 1. Each student is required to change out of their school clothes each day unless the teacher gives special notice. If a student refuses to dress, he/she will be referred to an administrator. 2. Passes or medical excuses should be given to the teacher during roll call. 3. All clothes used for PE should be kept clean and in good repair. They should be washed every weekend or as needed. 4. Each student has the option of keeping a non-aerosol non-spray deodorant product in his/her locker. 5. Any injuries sustained during class must be immediately reported to a Physical Education teacher. 6. Athletic shoes are required. They must provide proper support for a variety of physical activities. 7. All PE garments must be labeled with permanent ink. First name and complete last name are to be printed clearly on each item. No other markings on the garments will be allowed. 8. During cold weather, a sweatshirt, sweatpants, or warm-up outfit of any color may be used over PE clothes. Names should be on these items as well. 9. Lock valuables in your locker. The school is not responsible for lost items in the locker room. 10. Lockers must be locked when not being used. There will be a $4.00 charge for lost locks. 11. Students wearing any unsafe items will be asked to remove them before participating in PE. 12. During school hours, PE clothes are only to be worn during PE. DRESS REQUIREMENTS * 1. Students are required to change out of their school clothes and into PE clothes. Loaner clothing will be available for class use. Excessive use of loaners may result in detention and effect citizenship grade. 2. Students are only allowed to wear their own PE uniforms. 3. PE clothes should be worn in accordance with school dress codes. Shorts must be worn at waist level. No altering of shirts or shorts will be allowed. EXCUSES FROM ACTIVITIES 1. Limited excuses: The PE teacher may excuse a student for one day at a time. Requests must be made during roll call, but this is not an excuse from dressing. 2. Parental excuses: Parents can request from one to three days by note. In the note, the parent must state: o o o The student’s problem How long the excuse is to be in effect If the student is to dress or not 3. Long-term excuses: For any illness or injury that requires more than three days rest, an excuse from a medical doctor is required. The note needs to include the start and end date of the excuse. These excuses are to be presented during roll call. AN INJURED OR ILL STUDENT SHOULD NOT PARTICIPATE, BUT DAILY PHYSICAL ACTIVITY IS VERY IMPORTANT. Make-up of missed days or time will be based on the discretion of each teacher. 4. If a student is medically excused for twelve days or more during a trimester, adjustments to the student’s program will be made. 5. Health problems: Any specific health problems a student has must be reported in writing to the Physical Education teacher. Examples include: asthma, allergies, chronic knee problems, etc. Any long-term medical conditions must be reported to the school nurse and the Physical Education teacher. GRADING Every student will be given daily opportunities to earn participation points. Additional points can be earned for each activity for demonstrating physical skills, knowledge of the sport, and effort. Also, points can be earned by various actions such as: recording team results, helping other students, managing equipment, etc. Points can be deducted for failure to participate in daily warm-up activities or failure to participate appropriately in the class activity. The following grading procedure will be used: Participation = 50% Physical Fitness = 25% Skill and Written Work = 25% Extra credit will be available based upon the discretion of each teacher. Students can be given an improvement grade as part of their fitness achievement. PROGRAM Course offerings for all students may include: physical fitness, flag football, soccer, basketball, volleyball, tennis, team handball, health/first aid, softball, jump rope, track and field, tumbling, pickle ball, conditioning, dance, speed ball, over-theline, weights, aerobics, yoga, badminton, floor hockey, cooperative games, disc golf, lacrosse, speedminton, Socci ball, self-defense, Frisbee games and skills needed to develop a healthy lifestyle. GOALS AND OBJECTIVES Our goals and objectives will be based on the Physical Education Model Content Standards for California Public Schools. The five main categories are listed below. Students will demonstrate the motor skills and movement patterns needed to perform a variety of physical activities. 2. Students will demonstrate knowledge of movement concepts, principles and strategies that apply to the learning and performance of physical activities. 3. Students will assess and maintain a level of physical fitness to improve health and performance. 4. Students will demonstrate knowledge of physical fitness concepts, principles, and strategies to improve health and performance. 5. Students will demonstrate and utilize knowledge of psychological and sociological concepts, principles, and strategies that apply to the learning and performance of physical activity. DAILY PROCEDURE * 1. Each student is expected to be on his/her roll call position when the tardy bell rings. 2. Students have six minutes to dress. 3 Students should have everything they need for PE when they leave the locker room. The locker room will be locked and students will not be permitted to re-enter the area without supervision. 4. Class participation time is over when the teacher has excused their class. 5. Leaving an assigned area before dismissal, or being out of assigned teaching area during the period, may result in disciplinary action. 6. Students are expected to stay in a designated area before the passing bell. All other areas are off limits. 7. Be safe, responsible, and respectful in the locker room and bathrooms. Keep these areas clean. 8. No eating or drinking will be allowed in the locker rooms. 9. No changing of clothes in the restroom area will be permitted without prior approval. 10. If phones, camera phones, cameras, or any recording devices are seen in the locker rooms they will be taken by the teacher and given to the Vice Principal. * Violation of any procedures or dress requirements may result in detention or other disciplinary action.
