National Honor Society Nomination Requirements
Please return this sheet as the cover sheet for your packet. Please place the forms in the following order,
but do not check off the items:

1. A sealed nomination form filled out by a core subject teacher.

2. A copy of your transcript (The minimum GPA for memberships is 90)

3.A copy of your current attendance record (obtained from a clerk in the attendance office)

4. A signed form from an assistant principal stating that you have not disciplinary problems.

5. Two letters of recommendation from teachers, counselors, or administrators who are NOT sponsors of
any organization to which you belong.

6. A form (part of the packet) listing your extracurricular, co-curricular, and community
activities/contributions. (Mandatory community service does not count, i.e. hospital rotations, or court
mandated community service).

7. A proof-of-participation form (part of the packet-make copies for each activity), signed by coaches,
sponsors, mentors, and/or anyone in charge of the activities in which you are involved.

8. A typewritten essay (300 words) addressing the following prompt: How have I been a positive
influence in my community (school, neighborhood, city, church, etc.).
