FORM TO INITIATE ACTION ON NOMINATION OF EXAMINERS FOR PHD THESIS
Purpose of Form : To expedite the process of appointing external examiners for PhD thesis
for students, students are required to submit this form (duly signed by student & supervisor)
to the Ms Nurshida Bte Sappuan located at N3-02a-30, School of MAE (contact no:
67904366) within 3 months before they submit their thesis in a form ready for examination.
I) For PhD student & Supervisor
The student and supervisor shall discuss the thesis progress before completing this form.

Ms Nurshida Bte Sappuan for coordination within 2 week from the date indicated
below.
