DEAN’S VERIFICATION OF MATRICULATION
This form must be completed by students applying for graduate assistantships or
administrative assistantships in departments/offices where contracts are not prepared
by the
academic Dean of their graduate program. The signature of the Academic Dean,
verifying
matriculation, GPA and test scores, is required in order for employment and financial aid
papers to be transmitted to the requisite offices. This form should be submitted to the
Dean’s Office of the college that the student is attending.
