Indemnity Form
Participation in the Guam Chemistry Titration Competition is conditional upon the
completion of this Indemnity form.

I hereby give permission for my (son/daughter) to participate in the Guam Chemistry
Titration Competition. I understand that the organizers and assistants of the Guam
Chemistry Titration Competition will take all necessary and reasonable precautions and
care during this competition. I further agree to indemnify and hold harmless the
organizers and assistants of the Guam Chemistry Titration Competition, including the
competition venue at the University of Guam, from any accident, injury, loss, liability,
damage or cost, that might be incurred due to my son’s or daughter’s participation during
this competition, whether caused by the negligence of my son or daughter or otherwise.
I confirm that the participating school is responsible for having completed a Risk
Assessment pertaining to student involvement in this competition and is also responsible
for having taken the necessary steps to train competing students in the safe laboratory
practice required by the competition.
