Volunteer / Supervisor Agreement
VOLUNTEER AGREEMENT
I have read and understand the University of Kentucky Campus Community Emergency
Response Team Bylaws. I certify that I am 18 years or older and that all information
reflected in my volunteer application is correct. I understand that a national criminal history
background check will be conducted on all applicants based on the information provided. I
give permission for any still photography and/or video footage in which I may appear to be
used for University public relations and community outreach and for any other purposes
deemed appropriate by the University of Kentucky Police Department. I agree to participate
in any and all UK C-CERT activities as a volunteer and with the understanding there is no
additional compensation. In addition, I release any involved agencies and jurisdictions from
any liability related to UK C-CERT training, activities or active scene response.

SUPERVISOR AGREEMENT
I have read the bylaws and understand the listed conditions for membership for my
subordinate as a volunteer of the University of Kentucky Campus Community Emergency
Response Team and agree to allow service in such capacity.
