I am writing to let you know that on June 19, 2013 I attended the following event “Obesity is a Chronic
Condition: reducing Co-Morbidities through Effective Weight Management”, which was hosted by the
Midwest Business Group on Health. I did not pay the typical registration fee. As promised, I am are
writing this note, within ten days of the event, to let you know I attended this event. Essential details
are as follows:
* The fee ($150) was waived for me.
* Commissioner Bechara Choucair approved my attendance in advance.
* The event was educational. A copy of the agenda is embedded in the email below.

I am the only person from the Dep’t of Public Health that attended.
