College Visit Approval Form
Student must get approval 3 days prior to college visit day by having a counselor and all
teachers sign the form after an appointment has been scheduled with the institution.
Counselors will verify that academic performance, absences, and/or discipline are in
good standing prior to approving. A completed form must be turned in to Mrs. Bass on
the school day following the college visit.
