Instructions: This form should be used when requesting new or additional information to the building directory
located on the main floor of your building or when you are requesting revisions or corrections to existing
information. Please provide the complete name of the individual and include titles, degrees, and other designations
that you wish to appear on the building directory. Also, please specify the room or office number of each
individual. Attach extra sheets as necessary. Please note that space on the building directories is limited; SPO
reserves the right to manage the size of the list as necessary.
