Instructions:
The form is divided into three sections – Yearly Goals, Day-to-day Job Responsibilities and Clarkson Values.
Yearly Goals:




In this section, enter the agreed goals and select the rating from the drop down menu based on
performance against each goal.
List the challenges faced in achieving each goal and provide any additional information relevant to goal
achievement.
This section carries 33% weight of the “Overall Year End Review Rating”.
This section covers goal that would be assigned as part of Clarkson @ 125
Day to Day Job Responsibilities:




In this section, enter the key day-to-day responsibilities and select the rating from the drop down menu
based on performance against each responsibility.
Use the current job description as a guide to key responsibilities.
List the challenges faced in accomplishing day-to-day job responsibilities and provide any additional
information relevant to the day-to-day job.
This section carries 33% weight of the “Overall Year End Review Rating”.
Clarkson Values:



In this section, for each Clarkson Value, provide an example that demonstrates the value and select the
rating from the drop down menu based on consistent demonstration of each value.
This section carries 34% weight of the “Overall Year End Review Rating”.

Falling Behind: Meets most goal expectations but does not consistently meet all requirements; behavior in support of Clarkson
values may have to be reminded; shows some initiative to fulfill role obligations and to be part of the team.
Off Track: Does not consistently meet goal expectations and cannot be counted on to deliver quality work; behavioral support of
Clarkson values is not predictable; contributes as part of the team after being asked or must be reminded of role obligations.
