The JML PTA is trying something new. This fundraiser will support many of the school’s
activities and needs. We will wish your child a Happy Birthday during the month of their
birthday and raise funds for the school at the same time.
$10 per family.
The format will be HAPPY {MONTH} BIRTHDAY FIRST NAME LAST NAME only.
By filling out this form, attaching your $10 cash (or check made payable to JML PTA) and returning
it to the PTA during BTSNs or via the Front Office, you are authorizing the JML PTA to post your
child(ren)’s name & birthday month on the outside sign (to be seen by the public at large).
Parents can send this form and payment in NOW for any birthday month (Sept. 2015-Aug. 2016),
but the DEADLINE is by the 3rd Tuesday of the month prior to the birthday month.
*We will list all of Sept. 2015 birthdays by mid-Sept. (send in immediately/ASAP for September).
Only names of current JML students will be listed. If you have more than one child attending
JML this year, the $10 cost covers the listing of all their names (if desired and listed below).
