Take OIP letter (along with passport, I-94 card, current Form I-20 and proof of local address) to a Social Security Administration field office to obtain an application for a Social Security Number/Form L-676 (Letter of Ineligibility for a Social Security Number). As you fill out the application, tell the officer you only need the Form L-676 Letter of Ineligibility so you can apply for a Driver’s License. You should do this no more than 30 days before you apply for the Driver’s License. *OIP recommends you go to the SSA office located on the south side of the Loop, at 77 W. Jackson Blvd. This can be reached by taking the red line ‘El’ to Jackson. Step 2: Go to an Illinois Secretary of State Driver Services Facility. There are two locations in Chicago and five others throughout Illinois: o Chicago West Facility at 5301 W. Lexington St (only open Tues-Friday, 8-5:30) o Chicago North Facility at 5401 N. Elston Ave o Also in Bloomington, Carbondale, Champaign, Rockford & Springfield. Be sure to have the appropriate documentation, including: o Proof of automobile insurance o Acceptable identification documents as noted per Illinois Administrative Rules (see next page) Step 3: Take the Driver Services standard written, vision and driving exams. (Individuals applying for a Learner’s Permit need only take the written & vision exams. The Learner’s Permit restricts an individual to vehicle operation only when accompanied by an Illinois licensed driver 21 years or older.) Upon successful completion of the application process and the written, vision and road tests, a Temporary Visitor Driver’s License will be issued. A certified copy is a document produced by the issuing jurisdiction which has an embossed seal or an original stamped impression. Foreign birth certificates are accepted as "proof" if accompanied with any other item listed in Group B.
