2014 Annual Report Center for Health Policy and Health Services Research OVERVIEW 2014 was an excellent year for the Center – the best year in our history in terms of peerreviewed publications. We expanded the scope of our activity in two significant areas – health policy analysis and pharmacogenomics – and were able to support research and demonstration projects in a number of clinical departments in addition to those based directly in CHPHSR. We received funding for some significant new research projects while continuing work and publishing findings from previously-funded studies. The year featured publications in all of the top-tier journals read widely in our field (New England Journal of Medicine, JAMA, Health Affairs, BMJ) and presentations to prominent groups like the National Health Policy Forum. HEALTH SERVICES RESEARCH Investigators in CHPHSR continued work in five major program areas: diabetes, cancer prevention and control, pharmacogenomics and chronic disease, mental health services, and screening and treatment of chronic hepatitis. Highlights in the five areas included: Pharmacogenomics and Chronic Disease The Center has been actively involved in research to improve the tailoring of medications to individuals (i.e., personalized medicine) through the identification of the genetic predictors of treatment response (i.e., pharmacogenomics). Over the past decade, CHPHSR has been a leader in measuring medication use and relating this to patient outcomes pertaining to asthma, diabetes, hypercholesterolemia, hypertension, congestive heart failure, and depression; support for this work has come from grants from the National Institute of Allergy and Infectious Diseases (NIAID), the National Heart Lung and Blood Institute (NHLBI), and the National Institute of Diabetes and Digestive and Kidney Disorders (NIDDK) of the National Institutes of Health (NIH) and other sources, such as the American Asthma Foundation. Investigators in the Center have pioneered methods of combining electronic medical records data with high throughput genomics to perform large-scale pharmacogenomic studies. This work has already resulted in identifying a number of genetic predictors of asthma medication response. Research from these projects has also produced high-profile publications in The New England Journal of Medicine, Nature Genetics, Circulation, American Journal of Respiratory and Critical Care Medicine, The Journal of Allergy and Clinical Immunology, and Journal of the American College of Cardiology. Mental Health Services CHPHSR has been very active in psychiatric research in 2014. We remain core members of the Mental Health Research Network (MHRN). Our group has become a national leader in suicide research, including studies examining of patterns of health care use before suicide and suicide attempt as well as an investigation of suicide attempts/deaths before and after FDA warnings on antidepressants and continued publications on the Behavioral Health Services’ award-winning Perfect Depression Care initiative. We recently received funding to implement mental health, substance use, and suicide screening as well as proactive psychiatric consultation services within the Henry Ford Macomb Hospital. We’ve also collaborated with the Office of Clinical Quality and Safety to evaluate implementation of an online cognitive behavioral therapy focused depression intervention as an adjunct to usual care. We have continued our work studying treatment adherence among patients with depression, and have become active in research on the misuse of prescription opioids. In addition, as part of our continued collaboration with Behavioral Health Services, we now lead instruction of the research methods curriculum for all psychiatry medical residents. Our strong 1 collaborations have positioned us to participate in several other mental health services projects in the coming years. Chronic Viral Hepatitis CHPHSR is involved in two research studies, CHeCS and BEST-C, investigating chronic viral hepatitis testing, treatment, and outcomes. Both studies are sponsored by the CDC Foundation. The Chronic Hepatitis B & C Cohort Study (CHeCS) is a prospective, longitudinal observational cohort study created to assess the impact of chronic viral hepatitis in the United States. Henry Ford Health System, via a collaborative effort between CHPHSR, the HFMG Division of Gastroenterology/Hepatology, and the HFHS Public Health Sciences department, serves as the lead site and data coordinating center for this multicenter study. Among recent findings – that approximately 40% of CHeCS patients are susceptible to or have no documented immunity or vaccination against hepatitis A (patients with hepatitis B or C) or hepatitis B (patients with hepatitis C); also that 17% of CHeCS patients with hepatitis C already had advanced liver disease at the time of their initial HCV diagnosis, and these patients had higher long-term rates of hospitalization and mortality compared to patients diagnosed at an earlier stage. The BEST-C project includes a retrospective study and a prospective study. The retrospective study documented relatively low rates of hepatitis C screening, and the prospective study showed that rates could be significantly increased with a reminder system tailored to patients in the highest-risk age band. Diabetes Management Findings from the AHRQ-funded 11-site retrospective cohort diabetes study SUPREME-DM continued to be published in journals such as Annals of Pharmacotherapy and the American Journal of Epidemiology in 2014. The SUPREME-DM study has found that overall diabetes incidence has not significantly increased in recent years, but increases in hemoglobin A1c testing may have contributed to rising diabetes incidence among nonwhite patients. In addition, patients newly diagnosed with diabetes face an increased burden of medications used to control diabetes and other comorbidities as well as an increased focus on cardiovascular disease risk factor control after diagnosis of diabetes. Cancer Prevention & Control Several CHPHSR studies focused on cancer prevention and control in 2014. Findings from the observational Preventive Health Discussions study indicate that physician use of the 5 As communication technique (Assess, Advise, Agree, Assist and Arrange) when discussing colorectal cancer screening with patients is associated with increased screening uptake (Patient Education and Counseling, 2014). Results from a tailored intervention to increase colorectal cancer screening among African American patients found that patient communication preference moderated the effect of the intervention; patients with a more autonomous style were more likely to be screened following the intervention compared to others. HEALTH POLICY In March of 2010, CHSR was asked to take on the role of leading and coordinating health policy analysis for Henry Ford Health System. This decision was driven in part by the passage of the Patient Protection and Affordable Care Act (ACA), but also driven by the increasing number of opportunities for Medicare demonstration projects that might include HFHS as a participant. The specific tasks taken on by CHPHSR under the "Health Policy" heading include: monitoring of regulatory developments following ACA; monitoring announcements of CMS and other federal demonstration projects; coordinating HFHS applications to CMS and other demonstration projects; analysis of HFHS and other relevant data sets to have background information for HFHS communications on health policy; preparation of white papers and other forms of communication about health policy issues; attendance at health policy meetings and conference calls as HFHS representatives. 2 Two Research Associates provide staff support for the health policy activity. CHPHSR has played a key role in the preparation of several demonstration project applications based in other clinical departments of HFHS business units; for example, a CMS demonstration project by the HFMG Department of Radiology for a project on computerized decision support for ordering of imaging studies and the CMMI Bundled Payments for Care Improvement Initiative.application led by the Henry Ford Physician Network. Staff also conducted analyses and developed reports on the status of “dual eligible” (patients who are covered under both Medicare and Medicaid) in support of Henry Ford’s participation in a state-wide demonstration project about coordination of care for the dual eligible population.. In May of 2012, Dr. Nerenz of CHPHSR was appointed to the Medicare Payment Advisory Commission (MedPAC) as one of 17 Commissioners. MedPAC is an influential policy-setting body that advises Congress on a variety of payment issues in the context of the Medicare program. Dr. Nerenz also co-chaired an Expert Panel for the National Quality Forum working on the issue of adjustment of health care performance measures on the basis of socioeconomic and demographic factors. This is an issue of great significance to Henry Ford Hospital, other parts of the System, and providers across the country who serve economically and demographically “vulnerable” populations. NEW AWARDS Combined Transcriptomics and Genomics to Find Asthma Genes in Admixed Populations P.I.: Keoki Williams, M.D., M.P.H. Funding Source: NIH Funding: $2,911,122 (total cost) This project leverages both recent technological advancements in measuring gene expression (i.e., RNA‐sequencing) and the ancestral diversity of African American individuals to identify asthma‐related genes. By combining these transcriptomic and genomic approaches, we anticipate having greater statistical power to detect asthma genes and their regulatory elements, thereby identifying new biomarkers and therapeutic targets to better diagnose and manage asthma. Mental Health Research Network II Site PI: Brian Ahmedani, Ph.D. (PI: Greg Simon, M.D.) Funding Source: NIH (Subcontract with Group Health Research Institute) HFHS Funding Amount: $277,978 (total cost) Henry Ford Health System will work with other MHRN investigators to identify patient cohorts for future studies, conduct infrastructure work and preliminary analyses, gather other preliminary data needed to design MHRN studies, and will work with clinicians and others at HFHS to prepare for specific studies carried out by the MHRN. Integrating Behavioral Health Screening and Consultation into a Medical Hospital P.I.: Brian Ahmedani, Ph.D. Funding Source: Flinn Foundation Funding Amount: $200,000 (total cost) This project seeks to integrate proactive psychiatric consultation services into routine care within a large general medical hospital. This proposed intervention is an evidence-based mental health substance use, and suicide screening, assessment, treatment, and referral model. *Comparing Engagement Techniques for Incorporating Patient Input Site P.I.: David Nerenz, Ph.D. (PI: Jeffrey Jarvik, MD) Funding Source: PCORI (Subcontract with University of Washington) Funding Amount: $31,459 (total cost) 3 This project involves working with patients and patient advocates to identify the best ways of eliciting preferences for back pain care. The project will use quantitative and qualitative methods to compare ways of eliciting preferences, with the ultimate goal of making care more patient-centered and more responsive to patients’ balancing of risks and benefits. INPHAASE: A Temporal Data Driven Approach to Estimate the Risk of Heart Failure Events HFHS P.I.: David Lanfear, MD, Badri Padhukasahasram, Ph.D. HFHS Funding: $17,000 (total cost) In this project, we propose to develop novel computational algorithms that can efficiently utilize large volumes of genetic data and various longitudinal and clinical variables to improve and individualize risk prediction under the Cox Proportional Hazards framework. Developing such comprehensive and integrative models will be an important step towards maximizing the accuracy of predictive tests so that it can eventually become clinically relevant and bring us closer to the exciting possibility of personalized diagnosis, treatment and prevention. INPHAASE: Effects of the Affordable Care Act on Preventive Care Use and Disparities in Use HFHS P.I.: Jianhui Hu, Ph.D. (WSU PI: Wassim Tarraf, Ph.D.) HFHS Funding: $9,396 (total cost) Using both nationally representative data and data from Henry Ford Health System (HFHS), this study examines the national and local impact of the newly mandated preventive service benefits, enacted under the Affordable Care Act (ACA), on service utilization and healthcare disparities for such services.
