Ref. # List & describe the hazards and hazardous jobs Priority
Identify hazards by:
- Consulting with workers
1
2
- Observing work practices 3
- Reading labels and literature 4
- Reviewing incident and injury 5
reports
Examples:
- Carrying heavy materials
- Trip hazard in walkway
- Repetitive action without break
- Driving a long distance
- Using a hazardous substance
- Noise from machinery

Eye damage if corrosive chaotropic salts contact eyes 4
Poisoning if chaotropic salts ingested 5
Skin damage if chaotropic salts contact bare skin
Environmental damage if chaotropic salts disposed of incorrectly (eg. down sink)
Environmental damage if recombinant DNA is disposed of incorrectly (eg. down sink)
Injury due to incorrect use of centrifuge
Fire or electrocution due to centrifuge malfunction

For each of the hazards and hazardous jobs listed in Step 1, record a priority rating in the column beside that hazard/job after referring to the matrix below. The likelihood of injury will be reduced if effective risk control measures are already in place eg., safe operating equipment and procedures, trained staff etc.
Found a hazard? Think about: How severely could it hurt someone?
How likely is it to hurt someone?
very likely ++
!!! kill or disable !! several days
off work
! first aid could happen regularly
1 2 3 likely + could happen occasionally unlikely - could happen, but only rarely very unlikely -- could happen, but probably never will
