consult with Co-Chair of C&I to determine if the change is simply a point

Full review: Modifies the programs of more than one department or
program, or of the College as a whole, or is seeking a fully online (<80%)
program (Checking this box indicates a need for C & I approval and the
approval of the Faculty Assembly.)
This form is used to initiate any change in the instructional delivery mechanism (moving a traditional face-to-face
program to a hybrid or online format) within an existing undergraduate major, minor, or concentration. Complete all
sections. Signature of the department Chair overseeing the major, minor, or concentration is mandatory. Comments
and signatures of Chairs from affected departments are required. After a hard copy with signatures is given to the Chair
of C & I, this form should be electronically sent to the C & I committee Chair.

To increase the clarity of your proposed changes, please create a table that outlines the current program
requirements in the left column and the proposed changes in the right column. Wherever appropriate,
include subheadings, change in course hours, patterns of choice among options, etc. Be sure to distinguish
which courses are offered in a traditional format, in a hybrid format, or strictly online.

If approved by the Curriculum and Instruction Committee, the committee chair will submit the proposal to the Faculty
Assembly for a vote. If approved, the sponsor is to work with the registrar’s office and the provost’s office to prepare the
proposal for submission to the state for a HEGIS code (if required).
