This form is used to initiate any establishment of a new major, minor and/or concentration in an undergraduate
program. Complete all sections. Comments and signature of the department chair that would oversee the
major/minor/concentration is mandatory. Comments and signatures of chairs from affected departments are
required, as are comments and signatures from the School Dean, the Library Director, and the Registrar. This
form should be submitted with required signatures to the Chair of C&I, and an electronic version should also be
sent.

Briefly describe the proposal and explain its rationale:
Please explain the proposal’s academic integrity and its consistency with the mission of the college. In order
do demonstrate the proposal’s academic integrity, include one of the following:
1. Explanation of how the proposal aligns with accrediting body standards or professional organizational
standards, if available.
2. Submission of comparable programs from other schools with a description of how the proposal aligns
with comparable programs, and at least one letter of support from a recognized name in the field who
is not connected to St. John Fisher College.
If neither #1 or #2 are available or appropriate for your proposal, please contact the Chair of C&I for guidance.

After consultation with the Library, describe the strengths and weaknesses of library resources (print and
electronic). Provide specific recommendations for needed materials and projected costs. Have the Library Director
sign on the appropriate line on the last page of this form.

If proposing a hybrid or online program, please describe how you plan to adhere to accepted best practices in
instructional design that include the below elements. Please attach course syllabi or other materials.
a. Variety of interactive, practical, applied and innovative strategies to meet the diverse learning styles of
students; Activities are authentic, reflective, and responsive; Multidimensional learning environments

If approved by the Curriculum and Instruction Committee, the committee chair will submit the proposal to the Faculty
Assembly for a vote. If approved, the sponsor is to work with the registrar’s office and the provost’s office to prepare the
proposal for submission to the state for a HEGIS code (if required).
If approved by the Curriculum and Instruction Committee, the sponsor will work with the Hybrid Advisory Board to prepare
individual courses for approval (as appropriate for individual courses)
