Vehicle Ridership Form (#934) Instructions
Elderly- 60 and over
Disabled- A Mental or physical impairment temporary or permanent disability that limits your mobility
for using personal transportation.
General Public – riders which do not fall under Elderly or Disabled.
Contract Services – Expenses incurred by the agency for contracting out transportation services through
a third party. (Example: taxi cab or other private provider) KDOT requires a copy of the
contract at the beginning of each contract period (July 1) for all contract services in order to
pay any related expenses.
Other – each item listed must be itemized. Administrative costs ARE NOT allowable.
Project Income – Donations and/or fares collected on the bus that is directly attributed on the bus.
Coupon books for rides are considered project income.
