This application is for students enrolled in units administered by the Faculty of Architecture
Design and planning and is to be used after the advertised deadline for each session.
Should your application be approved, the DC grade is without academic penalty however
you are still liable for your fees.

PART B: STUDENT TO READ
Please provide a written statement outlining the reasons for failing to apply prior to the
advertised deadline and the unforeseeable reasons that have prevented you from
completing the unit. You will need to supply one or more of the following supporting
documents (original or verified at SAC or certified by an authorised witness).
Medical certificate(s)
Statutory Declaration for non-medical matters (ideally additional material should be
provided, for example – obit, order of proceedings at a funeral, travel itinerary)
Your statement must include details of your registration with disability services or any
special consideration applications previously lodged, this will allow the Faculty to take into
account your circumstances when making a decision.
Applications processing time is normally two to four weeks, however you must supply all
relevant material in support of the application at the time of submission, incomplete
applications will cause delays and may not be considered.
