LOCKER AGREEMENT
I request to be assigned a locker in the UC lower level.
I understand that I am subject to the following conditions:
_____ I agree to pay the fee of $50 for the school year (Fall 2013 & Spring 2014).
_____ I agree to pay the fee of $25 for one semester only.
_____ I agree to pay the fee of $25 for the summer semester only (May 19 – July 31, 2014).
_____ I agree to leave the lock assigned to me on my rented locker at all times and not
substitute it for one of my own.
_____ I agree that I am responsible for the condition of my locker and I will be held
responsible for any damage that I do and any fees necessary for the repair of my locker.
_____ I agree that Carnegie Mellon University, the University Center, nor any staff, faculty
or employee is responsible or liable for any loss or damage of property.
_____ I understand UC staff has the right to inspect my locker at any time without notice.
_____ I agree to clean out my locker by the last day of final exams.
