Parental Consent Form
The parental consent form should be written in a style that indicates it is the parent or legal
guardian who is consenting to allow the minor to participate in the study. The heading for this
form is Parental Consent Form. Follow the standard format for consent forms described on
page 39 except for the concluding consent statement, which should be as follows:
You are voluntarily making a decision whether or not to allow your child/legal ward to
participate. Your signature indicates that, having read the information provided above, you
have decided to permit your child/legal ward to participate. You will be given a copy of this
consent form to keep.
