Re-grade request form for STAT 350
If the assignment was returned as a hard copy, you need to staple this form on top of the
assignment and hand the packet to your instructor or your TA. In all other cases, submit this
form via Blackboard. If you are requesting a re-grade on a group assignment, please also
include the names (sections) of all of your group mates. If you submit the re-grade request form
more than one week after the assignment is returned, we will deny your request.
