Application Process
1. Complete application questions listed on the back of this form (not more than two pages); and up to two pages of additional
support material. Complete an application for each award for which you are applying. Return completed applications to the
Extension Office by Monday, September 15, 2014.
2. You do not need to submit your records unless you are applying for Record Book of Merit and/or Outstanding 4-H Member.
3. You may attach two additional pages of support material related to this award.
 May be a story, resume, records or a portion of a record, photos, clippings, etc.
 Format: 8 1/2" x 11", one sided, 12-point font recommended.
Interviews for county recognition will be held on Sunday, September 28, 2014, beginning at 2:00 pm, at the Muscatine County
Extension Office. Please indicate your availability for an interview that day.

Member may receive award only once.
ACHIEVEMENT ~ Recognizes 4-H'ers who have made a
contribution to improving family and community living through
their 4-H projects and activities.
AGRICULTURE ~ Recognizes 4-H'ers who have developed in self
and others knowledge and respect for agriculture and its
importance to the community, and who has applied the best
management practices.
AK-SAR-BEN ~ Recognizes 4-H’ers who are making significant
progress toward maturity and leadership through their 4-H
service for the advancement of the 4-H program in Muscatine
County.
CITIZENSHIP ~ Recognizes 4-H'ers who have demonstrated
excellent citizenship in most project areas.
EDUCATION ~ Recognizes 4-Hers who have educated others
through communication, workshops, leadership, and citizenship
activities within their project areas.
I DARE YOU ~ Recognizes 4-H'ers who demonstrate unusual integrity,
balanced personal development, and a willingness to assume
responsibility.
LEADERSHIP ~ Recognizes 4-H’ers who have demonstrated
leadership in most project areas
LIVESTOCK ACHIEVEMENT ~ Recognizes 4-H’ers who have
learned basic principles of animal science and demonstrated
knowledge of sound breeding, feeding, and management
practices.
RECORD BOOK OF MERIT ~ Recognizes 4-H'ers for well kept
records and a well organized record book.
OUTSTANDING 4-H MEMBER ~ Recognizes up to three 4-H
members for outstanding achievement and participation in 4-H.
MUSCATINE COUNTY YEAR-END BEST HORSE PROJECT- Must
be enrolled in the horse project for current year.
Recognizes a 4-H’er who has demonstrated outstanding
records, participation, and involvement in the horse project area
for the current year. A separate “Muscatine County Year-End
Best Horse Project” award application along with current year’s
horse records is required. Application due to Extension Office
by September 15.
