Yearly 4-H Summary
This summary should be filled out toward the end of the year. This is one of the places where
keeping a calendar will come in handy! This summary not only covers 4-H, but also activities
you participate in at school, church, community, and home.

Fill out the information at the top of the page being sure to use the correct information
for the 4-H year – Remember, your age will be whatever age you were on September 15.
List what you did with your 4-H club. How many meetings or other events did you
attend. Were there any project activities you participated in? Did you attend the 4-H
swimming party? Did you do anything to help others in your community?
Did you receive any awards this year? Were you recognized for club attendance? Did
your name appear in the paper for taking exhibit items to the county fair?

Did you give any talks this year at the club meetings or at school? Did you participate in
any communications events at the county fair? Were you interviewed on the radio?
What are your responsibilities at home? Do you chore outside or inside or both? Are
you responsible for laundry or dishes?
What extra things do you do at school – sports, class officer, and organizations?
Are you an active member of your church? Do you have a job? Do you volunteer time
at the local hospital, nursing home, school, church?

List the projects you have completed. If you want you can summarize any projects you
didn’t complete a project record on.
List any additional information including but not limited to: photos, clippings,
certificates or writing about how you feel about your 4-H experiences.
