SCANNING DEVICE:
A scanner will be MANDATORY for this course. Several of the assignments must be
scanned into your computer and submitted as a word document. Ensure that you have access to
a scanner before the semester begins.

STUDENT MEDICAL HISTORY FORM:
A student medical history form must be completed for each student by June 6, 2012 by
11:59 p.m. This form is required by the Physical Education Department for participation in
physical activity courses. This assignment/form is located in Course Content – Unit 1. Print out
the form, complete the form, including signature, scan it, and submit it as an attachment. You will
not receive credit for any assignments until the medical history form has been submitted correctly.
If the form is submitted after assignments are due, you will not receive credit for those previous
assignments. Scanning the medical form is a requirement. It must be sent inside Blackboard.
LAST DAY TO WITHDRAW:
Thursday, July 5, 2012, is the last day to withdraw from the course. If you are having
difficulty, contact the instructor. If you stop participating, but do not officially drop the course, you
will receive a performance grade of “F.” You may repeat this course only once after receiving a
grade, including “W.”

METHOD OF EVALUATION
Laboratory Assignments
20%
Laboratory assignments reinforce the topic of the unit, and are listed in two locations: the
individual Unit folders under Course Content, and under the Assignment tab. Ensure that you
check both locations for assignments. This is an “all or nothing” grade. Each lab will be graded
individually; however, all labs must be completed in order to have the potential to receive the full
20% lab grade. It is not a “pick and choose which lab I want to complete” grade option. Labs not
submitted in Blackboard Assignments will not receive credit, thus losing the possibility of the
20% point value. Please read lab instructions carefully; labs completed incorrectly will not receive
full credit. If you have trouble submitting your lab assignment, call the 24-hour Blackboard
support phone number immediately for assistance. Waiting less than two hours before the
assignment deadline to call for support may result in not receiving credit for your assignments.
Submit labs early to avoid losing points. You will not be allowed to fax labs or drop labs off to the
instructor’s office. Make sure you are aware of the assignment due date, which is listed on the
assignment. LABS WILL NOT BE ACCEPTED LATE.

Exams may be found under the “Assignment” tab (not the “Assessment” tab) on specified dates
and times. Information on exam covers all material covered within the Unit; I.e., PowerPoint
presentations, reading assignments, website assignments, laboratory assignments, podcasts,
etc. Nothing is allowed during the testing period – no books, notes, PowerPoints, etc. You are
answering the questions with no assistance, just as you would during a testing period for an onsite course.
The Midterm Exam covers Units 1, 2, 3, 4 and 5, and will be available online. The exam will be
available beginning July 5, 2012, at 9:00 a.m., and closing deadline will be July 6, 2012, at 9:00
a.m. No make-up exams are allowed.
The Final Exam covers Units 6, 7, 8, and will be available online beginning August 6, 2012, at
9:00 a.m., and closing deadline will be August 7, 2012, at 9:00 a.m. No make-up exams are
allowed.
Discussions

Students will have the opportunity to participate in Discussion Postings. During the
course of the semester, each student will respond to the first discussion topic, “Introduce
Yourself.” Thereafter, the student will choose five discussions in which to participate. The
requirements are as follows: three of the posting submissions must happen before July 5, 2012;
and at least one of the postings comes from Unit 6 (you have three choices for Unit 6 – Part I, II,
or III). Once the Discussion Assignment period opens, the student will have 48 hours to respond
to the topic posted by the instructor. In order to receive full credit for your posting: (1) your
answer must be specific to the topic, (2) be thorough to defend your position, and (3) must be
based on material you have read, heard, or watched. Discussions are available at 9:00 a.m. and
are due at 9:00 a.m., on the specified date. Choose you discussion participation carefully. Do
not post more than five (5), as your gradebook will record the first five ONLY (not including the
Introduction). If you post more than five (5), you are not able to follow the guidelines listed above.

Each student will prepare a written review on a selected piece of literature. The student
will research and read an article of their choice concerning issues or information about health,
wellness, or fitness. The student will then type a one page summary of the article. A conclusion
paragraph should be included at the end of the summary stating the student’s personal opinion of
the article and the relevance to the walking class. This assignment listed under the “Assignment”
tab in Blackboard, and complete instructions for this assignment may be found here. Assignments
may be turned in any time before due date. Deadline: July 16, 2012.
Workout Log/Attendance
25%
Each student will complete a record of their physical activity (both resistance and cardio).
These logs are found on my website and page 143 of the lab manual, and will serve as a
reflection of your attendance. In order for the student to receive credit for attendance for this
hybrid course, the student must participate in four hours total (two on-site and two off-site), per
week, in resistance and cardio exercise bouts. The on-site two hours will be on Mondays, from
6:00 - 8:00 p.m. (a discussion on this topic is later in the syllabus). The off-site time may be
accumulated per week, but, at the end of the week, must total two hours. This activity is to begin
the first week of school, and you will be responsible for recording on your log the first week of
school. Please make extra copies of these logs, as there may not be enough copies in the lab
manual to last you the entire semester. Don’t forget the total off-site time must be at least two
hours per week, coming from resistance and cardio physical activity. If total time per week of
physical activity does not total four (2) hours, points will be deducted. On-site absences will be
reconciled during the last log-check period.
After each exercise bout, the student will record the following information on their logs:
Resistance Log (printout from website): weight, sets, reps, and total time. The total time
you invest in resistance will be recorded in two places: (1) at the bottom of the vertical
column, on specified date, on your Resistance Log, and, (2) on your Cardio Log, in
Column #7. Add this time to your cardio time for weekly totals.
Cardio Log (143): the activity and the total cardio time invested. This time will be added
to the resistance time to have a total physical activity time to record on the cardio log in
the “Totals” column.

The goal will be to post all grades on quizzes, assignments, discussions, etc. within a 48-hour
period from the time the assignment was due.
It is highly recommended that each student set up their own, personal calendar under the
“Calendar” tab in Blackboard. This allows students to keep track of due dates, assignments, etc.
ON-SITE CLASSROOM POLICY
Dress:
Appropriate clothing for weight training is required. This would include non-restrictive
workout clothing and proper footwear (warm-up pants, shorts, t-shirts, and tennis shoes). No
jeans, slacks, cutoffs, belts, boots, or sandals will be allowed. If proper attire is not worn, you
may not participate, and you will be marked absent from the class. Tennis shoes are
MANDATORY.
Supplies:

Water bottles, watches, gloves, belt, etc., are optional
* The absence of supplies and proper clothing will impact participation grade.
Daily Sign-in Policy
Attendance is taken at the beginning of each class, and you will be expected to sign the
attendance sheet at that time. Upon entering the classroom, sign the attendance sheet and take
personal folder. You are allowed to sign in only when you are ready to begin your workout.
When class time begins, a “tardy line” will be marked, and all names below the tardy line will be
counted late.
Tardy Policy
Class begins promptly at the scheduled time. If you arrive 1-10 minutes after scheduled
class time, you will receive a tardy. After that time, the sign-in sheet will be taken up, and the
student will be counted absent for that class period. Three (3) tardies will equal one (1) absence.
Attendance, Participation, and Punctuality
This course is designed as an activity class. The class structure is designed to allow for
active participation by each student. If you attend class, you are required to participate. Lack of
effort, enthusiasm, or desire to improve, will result in a deduction from the attendance grade. Do
not attend class if you are not prepared to participate.
Attendance is taken at the beginning of each class and you will be expected to sign the
attendance sheet at that time. If you arrive to class one to 10 minutes beyond the scheduled
starting time, you will be marked tardy. Three (3) tardies result in one (1) absence. The sign-in
sheet is taken up at that time, and arrival any time beyond the ten (10) minute tardy time, you will
be marked absent.
In order to receive attendance credit for the day, you must stay for the entire class. If for
any reason you leave and do not finish the allotted time period, you will be marked absent for the
entire class period.
This portion of your grade is included in the 25 points that may be earned as described
under the “Workout Log/Attendance” on-line section of the syllabus. Classes that meet on-site
one (1) time a week will have one (1) “free” on-site absence. This absence may be used at the
student’s discretion. Thereafter, absences numbers two (2) and three (3) will result in 5 points
each being deducted from the 25 point total. With the fourth absence, regardless of make-ups,
the student will not receive credit for the class and will receive a grade of “F.” In addition, the
student will have the opportunity to make up one (1) on-site absence. Please see the instructor
to schedule a time and day. Make-up deadline is August 1, 2012.
You may repeat this course only once after receiving a grade, including a “Withdrawal”
grade.
Upon Review of This Syllabus
You Should
1. Read it again thoroughly. Make sure you are clear about the expectations of the course
and are willing to put in the time necessary to do well. Late assignments or excuses will
not be accepted. Remember, you are completing the same amount of work as the onsite classes. Time management is essential. You are required to use a scanner to
submit some portions of your work. In the beginning, this takes a lot of time. If you
are not familiar with using a scanner, get familiar ASAP or consider an on-site class.
2. Develop an emergency plan for submitting your work. Find out campus library hours in
advance so that you can use campus computers or scanners if you run into problems.
Know where the closest Kinko’s is in case the campus libraries are closed. Identify
someone who will be willing to assist you if you are not familiar with word documents,
scanning, or Blackboard. Computer problems are no excuse! All work must be
submitted inside Blackboard in the correct format for credit. Submit work early to ensure
you won’t lose points.
3. Print a hardcopy of the syllabus. Keep it in a place you can refer to often. Highlight
discussion & assignment due dates. Be sure to download an updated copy inside
Blackboard once classes start.
4. Purchase the required text for the course (see page 1 of syllabus).
5. Make sure your computer is ready to work! If you have pop-up blockers, you will want to
disable them. If you need further instruction, call the Distance Learning Dept.
6. Complete the Medical History Form inside Blackboard. The front and top half of the back
side must be submitted BEFORE any other assignment is accepted. If a physician’s
approval is needed, I will advise you of that necessity. You must complete the form &
physically initial/sign where required. Scanning this document is a requirement. If
you do not have access to a scanner, feel free to use the LRC on any of the 3 campuses,
go to Kinko’s (or a similar place), or borrow. The medical history form will only be
accepted inside Blackboard in Assignments. Please do not send an e-mail with your
form attached inside or outside of Blackboard. Be sure to physically sign & initial the
required spots!
7. Complete the Course Agreement, located in Unit 1, and submit.

and login. Click on the "My Courses" tab and click on your course listed in the My
Courses channel. It will take you to your Blackboard Course.
