The Planning Team established # of objectives which are used to measure our progress toward the mission. The information below presents evidence of progress for each of the objectives and the resulting changes for students. Objective 1: Insert your 1st Objective here Write your Objective 1 Summary of Evidence here. If possible, include baseline data and identify what year it is from. If you have no quantitative data, include other evidence. Be Honest! Although we would like all data to be positive, we must be honest and identify needs. Consider these types of data to show progress on your objectives. Test data o STAR o Site assessment data such as writing, benchmarks, fluency, reading, BPST, DIEBLS, Bookshop assessments, etc o In addition to overall school data, be sure to identify progress for subgroups. Survey data o Parent, student, staff Behavioral data o Suspensions, referrals, counselor visits, number of awards/achievements, etc Enrollment data o Site enrollment, number of PTA members, number of parents on campus, number of students in clubs/activities, etc Anecdotal data o What do you see happening/not happening as a result of your actions? o Changes in staff/student/parent morale? Objective 1 Conclusion: Write your conclusion for objective 1 here. *Your conclusion does not need to only address successes. Clearly stating where your needs are shows all stakeholders that you are continually working towards the commitments stated in your objectives & ultimately your mission. That being said…of course highlight your successes. Objective 2: Insert your 2nd Objective here Write your Objective 2 Summary of Evidence here. Objective 2 Conclusion: Write your conclusion for objective 2 here. Objective 3, 4….etc Conclusions At the end of the first year of implementation of School name’s Strategic Plan there is evidence of movement toward each of the # of objectives. Each bullet should be a quick conclusion of each objective…below are the district examples o Students are developing and beginning to apply 21st century skills as demonstrated by improved written communication skills and identify that specific skills are being developed at their schools. o The rate of students who were not proficient in English-language arts fell while those not proficient in math remained constant based upon STAR results. o The number of students with personalized education plans is growing, in part as a result of site strategic plans supporting their development. o Students, parents and staff report that most students consistently model the six character traits and this is supported by the reduction in suspension-related offenses at the school sites. Continued focus around the key areas identified in the plan –your key areas listed here (examples: instruction, resources, communication, personalization, technology, character, and parent engagement) – will result in continued progress toward meeting the # of objectives and ultimately our mission. **Be Sure to Attach Appendix A (your quarterly reports) Email the completed annual report (with quarter 4 report attached) to Robin Snider (rsnider@sanjuan.edu), Administrative Assistant to Donna O'Neil, Director: Assessment, Evaluation & Planning DUE DATE: Friday, September 14th, 2012
