Facilities & Administrative (F&A) Cost Waiver Request Form
UNMC negotiates with the federal government the rates for reimbursement of F&A (formerly
known as indirect) costs from sponsors. A cost waiver is defined as the acceptance of a rate
lower than the approved F&A rate. Rates prescribed by sponsor policy that are uniformly applied
to all university grant recipients are not included as waivers under university requirements. All
waivers must be documented and approved using this form. F&A cost waivers are granted on a
case-by-case basis.
