Health Informatics Practitioners
Questionnaire on Personal & Professional Development
This questionnaire is issued as part of a University research study to be conducted in
2006. It aims to collect data from health informatics practitioners on their views
about personal and professional development, the impact of policy and other drivers
on personal and professional development and explore these with a view to making
recommendations for the future.
The questionnaire is completely anonymous, participation is voluntary and you do
not have to complete it however if you are interested in the research area, you can
include your details so that you can then take part in a follow up interview. This
interview will examine your responses in more detail in an interview, location to suit
you, which will not last longer than 45 minutes.
Data collected will be used only for the purposes of this study, and will be destroyed
once the researcher has attained the MSc qualification on which this research is
based. Personal data is not required. If you submit the questionnaire by Email and do
not want any follow up, the information you submit will be treated confidentially
and anonymously and no link will be made for the purposes of data analysis and
interpretation between your Email submission and the content of your questionnaire.
If you begin to participate in the study, then change you mind, you can withdraw at
any time. You should signal at this time whether you just want to stop your
participation and the researcher can use data collected to date, or whether you wish
your data withdrawn also.
This forms seeks to understand your drivers, motivation and feelings, and as such
may not be a quick 10 minute exercise. There are 18 questions. Please allow up to 1
hour, which can be completed in small chunks as you can save the responses in
Word before sending the completed form back, or by hand and post

for which the researcher is very grateful.
