1.0 BACKGROUND
The Consultant was retained by the Department to provide Construction Engineering and Inspection
(CEI) services for (list Contract number, FPID number and Location Information.)
2.0 PURPOSE
This Supplemental Amendment is required to (brief task description).
3.0 SCOPE OF WORK
(BELOW VERBIAGE IS AN EXAMPLE ONLY)
To provide the following Environmental positions: Project Manager, Ecologist/Environmental
Specialist, Office Technician, Field Technician, and clerical. Will need the ability to prepare the
following types of Environmental Reports: Monitoring Reports, Construction Reports, Maintenance
Inspection Reports, etc.
4.0 CONSULTANT RESPONSIBILITIES
Consultant responsibilities shall remain the same as the Original Agreement and any Amendments
issued to date.
5.0 DEPARTMENT RESPONSIBILITIES
Department responsibilities shall remain the same as the Original Agreement and any Amendments
