Follow the procedures listed to submit the Addendum to the Three-Year Application for use of
Title I funds at a Subpart 2 facility serving neglected/delinquent students.
1.
The neglected/delinquent facility will complete only the sections of this Addendum to
Three-Year Application that have changed since the original application was submitted.
2.
The facility authorized representative needs to complete the signature page (page 2) and provide the needed information.
3.
The completed three-year application should be shared and reviewed with the Local
Education Agency (LEA) contact.
4.
The LEA contact, after consultation with the facility director/representative, will: a.
Ask for revisions to the application and return it to the facility for additional work, or b.
Complete the signature page (page 3), indicating LEA approval
5.
The LEA will email the completed three-year application to goodman_s@cde.state.co.us
and mail the signature pages to

As needed, yearly updates to the application can be made by submitting an addendum.
7.
This application will be in effect from July 1, 2014 – June 30, 2017 or until reauthorization of the Elementary and Secondary Education Act requires changes.

Be sure to begin this application process early to allow sufficient time for consultation with the LEA, needed revisions by the facility, and submittal by the LEA before the deadline of
June 30, 2014..

(2) Pursuant to the provisions of P.L. 107-110, Title I, Part D, Subpart 2 of NCLB, it is certified that all records pertaining to fiscal audit program evaluation will be maintained and made available to authorized officials of the state or federal government upon request. It is further certified that the above-named agency has, in effect, a department salary schedule, a policy to ensure equivalence in teacher, administrator, and auxiliary personnel, and a policy to ensure equivalence in the provision of curriculum materials and instructional supplies in the regular education program.
By signing below, the Local Educational Agency confirms that this Title I, Part D, Subpart 2 three year application has been read and approved by the LEA designated contact person or designee.

Addendum to Three-Year Application
The eleven elements and all parts (see below) need to be completed by the facility requesting participation in the use of funding for the education of students housed in facilities for neglected and/or delinquent youth.
PROGRAM SERVICES
1.
As appropriate, provide a description of the formal agreements to operate programs for neglected/delinquent youth between the local education agency, the facility, and alternative school programs serving youth involved with the juvenile justice system and/or social services.
2.
Provide a description of the research-based (proven to be effective) Title I educational strategies, activities, and services designed to individualize education plans and improve academic student achievement among children in the institution, particularly in reading and math.
3.
As appropriate, provide a description of how the program will involve parents in efforts to improve educational achievement of their children, assist in dropout prevention, and prevent the involvement of their children in delinquent activities.
4.
Provide a description of the efforts to ensure that facilities working with youth are aware of a child’s existing individualized education plan.

5.
As appropriate, include a description of how the facility will work with probation officers to assist in meeting the needs of youth.
6.
As appropriate, describe the steps taken to find alternative placements for youth who are interested in continuing their education but are unable to participate in regular public school program.

As appropriate, describe how Title I services will be coordinated with existing social and health services to meet the needs of students at risk of dropping out of school.

As appropriate, describe how Title I services will be coordinated with programs operated under the Juvenile Justice and Delinquency Act of 1974.

PROGRAM EVALUATION
11.
Provide an evaluation plan to measure student progress and determine the program’s impact on the ability of participants to: a.
accrue school credits that meet requirements for grade promotion and secondary school graduation. b.
maintain and improve educational achievement toward Colorado’s content standards as measured by the Colorado Student Assessment Program, particularly in reading and math. c.
make the transition into a regular school program or alternative education program operated by a local education agency. d.
complete secondary school (or secondary school equivalent requirements) and obtain employment or post-secondary education after leaving the institution.
